Providing Treatment and Care

The Clinic for Alzheimer’s Disease and Related Disorders (CARD) provides assessment and diagnosis of Alzheimer’s disease and related disorders for patients throughout BC, including care and support for the affected individual and their family. The team spans a wide range of disciplines, including geriatrics, neurology, social work, genetic counselling and geriatric psychiatry.

FAQs

Below are the answers to some frequently asked questions about the Clinic for Alzheimer Disease and Related Disorders.

To become a patient at the clinic, you will need a doctor’s referral. You can find the patient referral form here. Your doctor must fill this out and fax it to 604-822-7191.

Faxes are preferred but mailed in referrals or emailed referrals to ubcalzheimersclinic@vch.ca are also accepted.

You can call or email the clinic with any questions, but please note that you cannot receive an assessment without a doctor’s referral.
